For people living with hypertension, diabetes, cancer, and other NCDs, care often extends over many years and involves regular consultations, medicines, diagnostic tests, and follow-up appointments across different levels of the health system. Clear communication between health workers and patients is essential for safe and effective care, as illustrated by the experience of 52-year-old Yomi Bolaji from Ondo State, who has managed hypertension for 15 years.
